Pilot Physical

It is important for you to provide our clinic with all FAA required information pertinent to your current health status. Deferred or denied physicals are most often due to the pilot's failure to provide this information. Also, please remember to discuss with our Aviation Medical Examiner (AME) any health issues you fell could affect your ability to fly safely.

You are required to fill out your 8500-8 Form on-line by using http://medxpress.faa.gov/.

Suggestions for things you should do before your appointment that may expedite your certificate:

  • If you have had any change in your vision, check with your eye doctor before your physical and bring you eye glasses to your appointment.
  • If you have a Special Issuance, bring your letter from the FAA. Make sure you have sent all necessary medical information to the FAA related to your Special Issuance.
  • Provide to our AME all medical information required by the FAA letter for your Special Issuance.
  • Report to our AME if you have any of the Fifteen Disqualifying conditions.
  • Bring a list of all your medications, including the reason you are taking them, dosing information, and if you are experiencing any side effects.
  • Have regular checks with your family physician if you have a chronic health condition. For example, high blood pressure or diabetes. Make sure you have labs done within three months of your pilot physical.
  • Bring your Statement of Demonstrated Ability (SODA), if you have one.
  • Bring other documents related to your health that you might think necessary, especially if you have had a major health event or change in health since your last certification.

The Aviation Medical Exam fee is due at the time of service. We accept cash, check and credit card. Our team realizes how important it is for you to get your certification. If an issue should surface, our AME will work with you, the FAA, and your personal physician toward resolution. It is our desire for you to leave our clinic with your certification in hand.
