Attention Anthem patients:
Effective April 13, 2012, Joint Township District Memorial Hospital, Grand Lake Home Health and Grand Lake Hospice are no longer a part of the Anthem Network. This does not include employed physician offices, which have a separate contract with the Anthem Network. We are continuing to wait on the Anthem Network to negotiate a new contract and have implemented a plan to help minimize the impact to our patients and to treat all patients and payers equally.
