Transitional Care Unit Celebrates it's 20th Anniversary

transtional-care-servicestransitional-careThe Transitional Care Unit at Joint Township Hospital, opened in 1990 to serve Auglaize and surrounding counties, is a 19 skilled nursing unit designed to meet the skilled needs of patients whether it be nursing and/or therapy. Located on the 6th floor of Joint Township Hospital, the Transitional Care Unit has provided quality care to over 10,000 patients for the last 20 years.

The longevity of our staff is a testimony to our continued great care and work ethic. We have team members that have been here since the opening and currently have a combined total of 217 years of employee service. Our team approach involves physicians, nurses, and care management identifying patient care needs and providing additional support as needed such as occupational, physical and speech therapy, respiratory therapy, laboratory services, diagnostic services, and nutritional services. One of the benefits that TCU has been able to offer over the last 20 years is providing services needed at a moments notice.

Since the opening of the unit, and over the years with healthcare changes, we have continued our commitment to quality care. Annually, for the past 20 years, TCU undergoes an inspection by the Ohio Department of Health to evaluate the care, and we are proud that our status has been in the top rankings and ranked in Ohio’s top 5% for patient satisfaction.

Recently with support from our community, the unit has been completely remodeled with an upgrade to our nurse call system in addition to a beauty shop and an apartment to allow for therapy to work with patients on safety needs and homemaking skills. In 2009 we completed construction on our beautiful outdoor patio for patient and family enjoyment. The TCU offers a full size gym to assist our patients in working with therapy to reach their maximum function.

Over the years we have seen a growth of new services on our unit. More currently we want to highlight some clinical changes, such as our ACT team, which provides assistance to the nurse in assessing a patient who is experiencing an acute change in his/her condition. The TCU also screens all patients to provide additional support of our Palliative Care Team as well as offering preventative diagnostic testing.
